Fieldquests Consumer Health Data Privacy Notice

Effective October 14, 2026

Effective date October 14, 2026. This notice explains how Fieldquests LLC, operating Fieldquests, handles consumer health data in optional learner care and accessibility notes. It supplements the Privacy Policy and applies where consumer-health privacy laws cover our processing, including in Washington and Nevada. Privacy contact karen@fieldquests.com. Mailing address available upon verified legal request.

Information sources and purposes

An authorized adult may provide allergy, disability, medication, accommodation, or other relevant participation notes linked to a learner and booking. Relevant health information may also appear in a safety report, accommodation discussion, or incident record supplied by a guardian, authorized group leader, assigned teacher, or responder. We process these records electronically to arrange the requested participation or accommodation, support class safety, respond to incidents, and meet applicable legal duties. Do not submit full medical records or unnecessary diagnoses. Group leaders maintain individual records onsite; count-based enrollment does not require routine uploading of every learner’s health information.

Choices access and deletion

Use the learner profile or contact karen@fieldquests.com to review or correct notes, withdraw collection or sharing consent, or request deletion. We reasonably verify identity and guardian authority. Withdrawing sharing permission removes routine teacher access; deleting notes is a separate choice. Routine teacher access also ends after the class or booking cancellation. Records necessary for a specific lawful incident or legal purpose are separately restricted; retention exceptions apply only where the governing law permits. Active care notes are kept only while needed for the learner profile, an authorized booking, accommodation, safety, or a specific legal purpose. Covered archived and backup copies are deleted within the governing deadline and no later than six months where Washington law applies.

Additional rights where applicable

You may confirm processing, access covered health data, withdraw consent, and request deletion, including required notices to recipients. Washington requests may include a list and contact mechanism for third parties and affiliates receiving the data. We respond within the applicable legal period, generally 45 days, and explain any permitted extension or denial. Appeal a denial at karen@fieldquests.com. We explain the appeal outcome and available complaint process, including contacting the Washington or Nevada attorney general as applicable. Required deletion includes backups within the governing deadline; Washington allows no more than six months after authentication for archived or backup deletion. We do not unlawfully discriminate for exercising privacy rights.

Changes and contact

We post an updated effective date and notify affected adults of material changes through account or contact email. New health-data categories or purposes require additional disclosure and consent where required. Contact the privacy channel above with questions. A prominent separate link to this notice must appear on the homepage and where care notes are collected: https://fieldquests.com/policies/consumer-health-data.
